History: Martha Wilmington, a 74-year-old woman with a history of rheumatic fever while in her twenties, presented to her physician with complaints of increasing shortness of breath (â€Å"dyspnea†) upon exertion. She also noted that the typical swelling she’s had in her ankles for years has started to get worse over the past two months, making it especially difficult to get her shoes on toward the end of the day. In the past week, she’s had a decreased appetite, some nausea and vomiting, and tenderness in the right upper quadrant of the abdomen. On physical examination, Martha’s jugular veins were noticeably distended. Auscultation of the heart revealed a low-pitched, rumbling systolic murmur, heard best over the left upper sternal border. In addition, she had an extra, â€Å"S3† heart sound. Using the internet, do any applicable searches to give a reasonable scientific explanation to the questions below. Perhaps there has been narrowing of Martha’s pulmonary semilunar valve which is located between the right ventricle and the pulmonary artery. The closing of this valve is heard best over the left upper sternal border. As stated above, this is where auscultation of the heart revealing a low-pitched, rumbling systolic murmur. A murmuring sound is heard due to the high resistance to blood being pumped through. 2) What is causing her â€Å"S3† heart sound? An S3 sound is an extra sound indicating abnormal blood pressure within the heart, namely against the ventricle walls during diastole (relaxation). Blood seems to be flowing too rapidly into the ventricles during diastole. She may have ventricular walls that have become hardened and thus not relaxing as needed in order to fill with ease. As the blood quickly flows in, it will hit the hardened walls, creating an extra sound. In congestive heart failure, preload and contractility are major factors in the improper functioning of the heart as a pump. 3) Is her history of rheumatic fever relevant to her current symptoms? Explain. Rheumatic fever caused by Group A Streptococcus bacteria may cause damage to heart tissues including valves. Overtime, congestive heart failure may have developed. However, the pulmonary semilunar valve seems to be the issue in this case study, whereas rheumatic fever normally affects left heart tissue. 4) A chest X-ray reveals a cardiac silhouette that is normal in diameter. Does this rule out a possible problem with Martha’s heart? Explain. No, a normal diameter of a cardiac silhouette does not rule out a problem with Martha’s heart. The heart adapts and will compensate for damage in order to still function optimally. The right ventricle, in this case, will become stronger in order to push the same amount of blood (stroke volume) through the narrowed pulmonary semi-lunar valve. This thickening doesn’t necessarily change the inner diameter. 5) You examine Martha’s abdomen and find that she has an enlarged liver (â€Å"hepatomegaly†) and a moderate degree of ascites (water in the peritoneal cavity). Explain these findings. The increased resistance of blood flow through the pulmonary semilunar valve from the right ventricle backs up the pressure of blood flowing into the right atrium. This back flow pressure builds up in the body as systemic pressure, increasing hydrostatic pressure which increases fluid build-up (ascites) in the peritoneal cavity and liver, enlarging the liver. 6) Examination of her ankles reveals significant â€Å"pitting edema.† Explain this finding. Fluid builds up in the interstitial space of her extremities (i.e. ankles) due to the changes in the hydrostatic pressure caused by the back-flow pressure originating in the heart. 7) She is advised to wear support stockings. Why would this help her? Support stockings could be used. I worked at a Med-Spa and the doctor would advise his patients to wear compression hoses after laser vein treatments if there were no other individual contraindications of use. Compression hoses prevent fluid from accumulating in surrounding tissues and interstitial spaces by directing excess fluid into other blood vessels and the lymphatic system. 8) Which term more accurately describes the stress placed upon Martha’s heart — increased pre-load or increased afterload? Increased after-load describes the stress placed upon Martha’s heart. Afterload is the pressure that the heart pumps blood against. There is increased pressure associated from left ventricular damage and narrowing of the semi-lunar valve in Martha’s diseased state, leading to the resistance of blood flow. 9) What is the general term describing Martha’s condition? Right-sided congestive heart failure is the general term for Martha’s condition. 10) How might Martha’s body compensate for the above condition? Martha’s sympathetic nervous system will begin to predominate, causing constriction of blood vessels so that important organs will regain necessary delivery of blood. Also, heart rate and stroke volume affect cardiac output. In Martha’s case, cardiac output is affected due to a decrease in stroke volume. Higher resistance to blood flow decreases the amount of blood pumped by her ventricles, so the heart compensates in order to function optimally. Increased contractibility or the increased rate at which her heart pumps is indicative of this compensation. 11) Martha is started on a medication called digoxin. Why was she given this medication, and how does it work? Digitoxin (digitalis) increases the strength and length ventricular contraction which slows heart rate by reducing sympathetic activity. Her right ventricle will then pump more efficiently. 12) 12. Two weeks after starting digoxin, Martha returns to the physician’s office for a follow-up visit. On physical examination, she still has significant hepatomegaly and pitting edema, and is significantly hypertensive (i.e. she has high blood pressure). Her physician prescribes a diuretic called furosemide (or â€Å"Lasix†). Why was she given this medication, and how does it work? Digitoxin is commonly given in conjunction with a diuretic. The use of a diuretic manages edema, a symptom of congestive heart failure. Increased urination will rid her body of excess ions, reducing the amount of fluid in the interstitial spaces.

We are first introduced to Mary as being an independent young woman. However Lessing’s character soon shows signs of being an insecure woman, who cares deeply what other people think about her. The reader is forced to sympathise with this self-destructing character. Throughout the novel Mary is described as being in a state of tension and under strain. Mary is unable to adapt to her new life on the farm with Dick, she is constantly longing for the town she left behind. The linear plot is about Mary Turner’s life, going back to her childhood and progressing to her characters fatal ending. The narrator tells of Mary being raised by â€Å"frustrated parents† and the hatred she felt towards her father. Her body is treated with discust,†She smelt the thick stuff of his trousers†, a possibility that some sort of child abuse occurred, which would account for her arrested sexuality, the fear and repulsion of sex. Mary becomes a friendless character who receiv es no help from her Husband and no loyalty from the servant. However violent Mary becomes with her servant she never actually commits a crime. Mary is driven to marry Dick after she over hears people mocking her and she feels she is being ostracized. The reader views Mary as a heroine who has lost her struggle. We are told by the narrator that evil was not contained within this woman but that evil was all around her. Throughout the novel the author’s disapproval of sexual and political prejudice and the colonialism in South Africa is constantly reinforced. This in turn influences the reader not to adapt to the main characters viewing of the world. Lessing’s novel can be seen as Mary’s constant struggle to preserve her authenticity and sense of self but she fails to overcome her struggle due to the forces and conditions that surround her. Mary’s failures are rooted in her family and culture that in turn dooms her to her death. Although at the beginning of Mary and Moses’s relationship, Mary exerts all her power and authority, we soon see a role reversal and a curious relationship develop when Moses insists on being treated like a human. From the beginning of the novel we become aware of Mary’s family struggles of poverty. Lessing intentionally tried to make the reader constantly switch from sympathising with Mary to despising her. Both Mary and Dick are identified as being tragic figures because of their failure to communicate and to address the practical and emotional difficulties in their lives. Mary  believed that she was as a white person is superior to the black natives in every way. The relationship that Mary develops with her black servant Moses shatters the complacency of the whites in Africa. Moses’ power in the relationship is unquestionable and real. His action in murdering Mary is simply a demonstration of the control which he exerts over her and in general which the blacks have in their own country still. The whites only retain a hold based on lies and corruption The land is what kills Mary. Mary’s efforts to assert her white authority over a black man continually backfire and leave her with less control. â€Å"While it is never explicitly stated, the novel suggests that Mary succumbs to him sexually just as her mental faculties begin to disintegrate†(40) Mary’s cognizance of the murder as one compounded  by her own guilt and by vengeance, rather than unwarranted aggression, shows a strange ability to forgive her own murderer even as he performs the act that she knows he is compelled to do.(42) Theshadow of regret, followed by the desire to explain and to be absolved of guilt, marks the first and only moment in the novel in which Mary is conceived as a self-possessed agent of her own destiny(43) The reader never consent to Mary’s view of the world but they can relate to the traditions and cultures that she was raised in that influenced her behaviour. Mary had been brought up to be afraid of black men: â€Å"She was afraid of them [the natives], of course. Every woman in South Africa is brought up to be. In her childhood she had been forbidden to walk out alone, and when she asked why, she had been told in the furtive, lowered, but matter-of-fact voice she associated with her mother, that they were nasty and might do horrible things to her†(chapt4) â€Å"She hated their half-naked, thick-muscled black bodies stooping in the  mindless rhythm of their work. Bass and Riggio (2005) suggested that transformational leadership assist followers to grow and develop into leaders by responding to individual followers needs, by empowering them, and aligning the goals of the individual followers. LEADING DURING CRISIS Barack Obama got sworn into office during the time of an economic crisis. The country was undergoing a recession but nonetheless, he came into office ready to take action. One of his first priorities after getting into office was to tackle the financial crisis. His stimulus package and some of the carryover from the Bush administration pushed money into the economy when it was needed the most. His auto bailout gave manufacturing a lifeline (GM, Chrysler, Ford), and his support for credit starved banks, slowed down the spread of subprime contagion (Barmak, Beer, Brearto, Castaldo, Cowan, 2012). Even though some believe he did not make the economic life better in his first four years in office, he certainly prevented it from getting much worse. And on re-election day, voters rewarded him for that. According to exit polls, more than half of voters still blamed George W. Bush for the sluggish economy. Only 38% blamed the president (Barack Obama). Surprisingly, four in ten people told po llsters they believed the economy was getting better. Among that cohort, 88% sided with Obama (Barmak, Beer, Brearto, Castaldo, Cowan, 2012). In the last full quarter before the second election, real GDP in the U.S. grew at a slow rate of 1.3% and Joblessness remained very high. No sitting president had ever won re-election with unemployment above 7.4% in the months leading up to a vote (Barmak, Beer, Brearto, Castaldo, Cowan, 2012), but Barack Obama was able to pull it off. POSTMODERNISM According to research, younger generation in the United States and throughout the developed world now possess a more postmodern and post-materialist value system. For the first time in American history, there are four generations co-existing in the workplace. Which are, the Greatest Generation (1922-1945), the Baby Boomer (1946-1964), Generation X (1965-1980), and Millennial (1981-2000) groups (Green Roberts, 2012). It became clear that Obamas campaign communication methods and message appealed to the Millennias sense of community involvement. Barack Obama used the opportunity to connect with his generation and the younger voters population. The Bush administration was not able to meet the new set of values and expectations that the millennial generation is driven by. Some core postmodern traits include challenging authority, attacking conventional wisdom, tolerating ambiguity, accepting diversity, and building constructive reality (Green Roberts, 2012). Consequently, postmodernist s find themselves distrustful of institutions and hard facts. Determining the Ratio of Circumference to Diameter of a Circle   Ã‚  Ã‚  Ã‚  Ã‚  In determining the ratio of the circumference to the diameter I began by measuring the diameter of one of the si objects which contained circles, then using a string, I wrapped the string around the circle and compared the length of the string, which measured the circumference, to a meter stick. With this method I measured all of the six circles. After I had this data, I went back and rechecked the circumference with a tape measure, which allowed me to make a more accurate measure of the objects circumferences by taking away some of the error that mymethod of using a string created.   Ã‚  Ã‚  Ã‚  Ã‚  After I had the measurements I layed them out in a table. The objects that I measured were a small flask, a large flask, a tray from a scale, a roll of tape, a roll of paper towels, and a spraycan.   Ã‚  Ã‚  Ã‚  Ã‚  By dividing the circumference of the circle by the diameter I was able to calculate the experimental ratio, and I knew that the accepted ratio was pi. Error is the deviation of the experimental ratio from the accepted ratio. After I had the error I could go on to find the percentage error. The equation I used was, error divided by the accepted ratio times 100. For example, if I took the error of the experimental ratio for the paper towels, which was 0.12. I took that and divided it by the accepted ratio giving me .03821651. Then I multiplied that by 100 giving me about 3.14.
